Tiffanny goes into patients' homes to care for them and help transition them from hospital care to home care. There have been many instances that she has gone above and beyond caring for her patients. One instance in particular we had a patient in Hospital at Home that did not have a way to get her diabetic supplies from the pharmacy. Tiffanny ensured that the patient had all of the supplies she needed before being fully discharged from our program. Even after the patient was discharged and not in our care Tiffanny continued to call the patient to ensure she had all of the supplies she needed. She even made a run to get her some more supplies when she needed them. Tiffanny truly goes above and beyond to ensure her patients have the best care possible, especially those patients who are not fortunate enough to have caregivers to help.
