Maureen Kovchok is a Case Manager Preceptor for Team C63 who consistently demonstrates excellent clinical and compassionate care. She acts as a role model and preceptor to our nursing team, sharing her knowledge of home care and clinical expertise.
In my two years managing Team C63, Maureen has trained eight new VNA nurses, all who have thrived in their positions through her mentoring knowledge and skills.
Maureen manages her cases and cares for her patients following a holistic method. She is great at keeping her patients safe and knowledgeable in the community utilizing other disciplines such as Therapists, Social work and HHA to assist her.
A little over one year ago, Maureen cared for an elderly woman in Atlantic Highlands who lived with her ill and obese son. By collaborating with doctors and social work, Maureen was able to assist the patient, creating the best and safest care in the home. After many weeks of her efforts the situation started to improve, even though initially the son was not willing or able to care for his mother properly. Maureen also reached out to an estranged daughter who became involved and helpful to the care of the patient.
Unfortunately, during one visit Maureen found the son dead in the home with the patient in the next room. Maureen acted wisely and efficiently calling her manager and social work for assistance. Maureen orchestrated the entire plan and did an outstanding job even with multiple obstacles, such as when the funeral director could not remove the son from the home due to his obesity. Maureen was in the home for hours making sure the patient was safe and assisted the daughter with social work who transferred the patient to a nursing home later that evening. Maureen stayed with the daughter the entire time... She even helped to clean the house as it was cluttered and filthy.
I truly believe Maureen should be nominated for the DAISY Award. In all of my 35 years of nursing, this story will be a stand-out in my mind. Maureen Kovchok truly makes a difference in the lives of our patients and follows our VNA mission and core values.
