Kim Jacobs is much deserving of the DAISY Award. She consistently performs at a very high level, raising the bar for those around her. She has the ability to touch the lives of patients in most units of the hospital. From the Emergency Dept. to the PACU and especially the Units, Kim helps to ensure that the patients at St. Luke's receive the highest quality care every time.
I have witnessed her removing infiltrated I.V. access and establish newperipheralI.V. access during bedside rounds. Her level of caring goes beyond what is expected. Her care extends beyond the inpatient experience.
Each day, she will make sure to contact each patient's family to update them on the status of their loved one. She will follow up with patients after discharge to make sure that our treatment plan is going well. At times, she will identify instability and make recommendations or coordinate more rapid follow-up which prevents readmissions.
She is generally a delightful person and is the head of our Morale Committee, ensuring that all those around her are appreciated and recognized. She does so much for our patients and coworkers, I feel that she is deserving of recognition and celebration for her extraordinary compassion,skill,and dedication.
