I had the honor of working with Jessica recently while she assisted the Complex Case Management Team. This case involved a gentleman who was living in a 1-bedroom apartment with his daughter and her 2 small children. He was admitted to SJMC due to AMS. During this stay, his daughter stated she was no longer able to care for him in her home.
Jessica worked with this patient and his daughter to discuss other options. She discovered his family consists of a sister in Australia, an estranged wife who lives across the country and 2 daughters, 1 in Tacoma and 1 in Texas. All of them had tried to assist the man. Each had paid either for his living expenses or had him live with them, but all their options failed. While hospitalized, it was discovered he did not have the funding or coverage to pay for his own care. The patient was unable to recall how much income he received or where it came from.
Jessica very patiently worked with this patient contacting his bank and Social Security to verify his income and where it was being sent. Multiple calls had to be placed to various agencies to clarify what funds he received and where they were being deposited. Thanks to her persistent follow-up, his income was located. Unfortunately, she had to break the news to this patient that someone had been stealing all his income. It was obvious that this patient was going to need ongoing assistance with his medical and financial needs.
His daughter in Texas requested to be his Power of Attorney and provided the needed paperwork. Jessica was able to arrange a notary to meet with the patient the same day we requested it. Thanks to all her coordination, his daughter in Texas became his POA. When discharge arrangements had been finalized, Jessica helped troubleshoot problems with pharmacy coverage of diabetic supplies and ultimately arranged with our pharmacy to provide the additional items he needed. She assured all his discharge plans were complete and correct the day before discharge and was quickly able to fix last-minute changes in medications. She even arranged clothes for him at discharge as he had none with him. Jessica consistently, quickly, and kindly answered and followed up on the multiple requests the Complex Care Management Team presented to her. She exemplifies how a team should function.
