Debbie Quam is an exemplary representative of the superb care provided by our Patient Care Coordinators. She is the first to arrive at work each day and stays until the job is completed. She takes great care to communicate all patient needs to her coworkers daily in a clear and consistent manner.
Deb brings a wealth of experience to her role from direct patient care to policy and advocacy at the level of the Federal Government. As a discharge planner she takes into account all aspects of a patient's physical and emotional health when planning for post-acute care and transition from the hospital setting. Deb takes a leadership role meeting daily with other members of the multidisciplinary discharge planning rounds team. She coordinates care with physicians, nurses, social workers, physical therapists, pharmacist, nutritionists and chaplains. She also works closely and effectively with outside care providers such as skilled nursing facilities, pharmacies, home health and hospice. She is always professional while presenting in a warm and genuine manner.
Deb reaches out to make compassionate connections with our patients and their extended family members. She shows respect for the diverse needs of our patient population. Deb is able to be creative and responsive to the unique needs of each patient and family.
This nomination is made on behalf of the San Rafael inpatient Social Work team who is honored to have the opportunity to work with a staff of competent and caring Patient Care Coordinating Nurses. Deb, in particular, exemplifies the empathy and caring attitude that we appreciate in all of the team.
