Leah worked diligently to grant the patient's wishes by coordinating conversations between hospice nurse, family, and to ensure that all involved knew what to expect.
Leah had a patient that wanted to go home with Hospice. Although it was the patient's wishes, the family had difficulty coming to terms with this impending prognosis. Leah spent numerous hours with the wife consoling her and just being there to let her talk. Leah also made arrangements for the family to come up together so that they can spend time with the patient before he drifted into the coma. She went out of her way to ensure that the AOD approved, called hospice personally to come in and speak with the wife coordinated this care carefully so that his family would walk in and see that he was now comfortable.
Leah worked diligently to grant the patient's wishes by coordinating conversations between hospice nurse, family, and to ensure that all involved knew what to expect. Leah also spoke to the physician to ensure that the patient was comfortable while waiting for transport for him to go home. Leah gave the patient's wife helpful suggestions on how to turn the patient, how to use draw sheets, and more importantly to not be afraid that she will be available for anything that they might need while the patient is in the hospital.
The Hospice nurse that was here to set up everything told me what a great asset Leah was to the family as well as to Hospice and how caring, understanding, and what a great job she did with the patient. I was so touched by how Leah showed compassion care, understanding with the family, but most importantly she ensured that not only was the patient comfortable but that he got his wishes.
