Annmarie collaborated with therapy, social work, nutrition, and HHA services to give Mr. D the best quality of life possible.
Annmarie Keenan is an Extraordinary Nurse who works as a Full-Time Admission Nurse/Case Manager for Team C63. She is an experienced Community Health Nurse who began working at the VNA in 2002. It has been a pleasure working with her over these last 5 years and I am very grateful to have her working for Team C63. She has strong clinical skills and experience that enable her to provide best practice and quality nursing care to our clients. The 3 strongest characteristics I selected for Annmarie are 1- engages in efforts to promote and restore the health of individuals, 2-delivers holistic care encompassing mind, body, and spirit, 3- collaborates with the individual patient and family, and fosters communication.
This past year Annmarie has been caring for Mr. D a 54-year-old male with CVA, s/p craniotomy, trach, peg tube, Foley, non-verbal, and bedridden. Mr. D transferred from Ohio to NJ at the height of the pandemic. His wife in Ohio could not provide for him and so the sister in NJ offered to be his primary caregiver. As you can imagine this case was/is challenging medically, socially, and emotionally but because of all the efforts put forth by Annmarie the patient has remained home receiving excellent care. In the beginning, when Mr. D first came onto service there are many, many obstacles such as high acuity of care, no primary MD to coordinate with, insurance issues leading to charity, and self-pay. But Annmarie took each of these challenges in stride and conquered them all. The sister became proficient in trach care, tube feedings, foley care, oxygen safety, suctioning, medication management, and kept a bedridden patient free of pressure ulcers. Mr. D has been without a pressure ulcer since coming onto service in March of 2020 and has never had a re-admission to the hospital.
