Michele goes above and beyond to try to simplify the often-overwhelming instructions appointments and medications that patients are left to try to navigate and coordinate. She spent over an hour recently with a patient who had been discharged from the hospital where he received two new diagnoses, several new medications, and numerous appointments with various specialists. He was confused and agitated but was reassured and relieved after meeting with Michele. She not only validated him, but she deconstructed his discharge paperwork. She set him up for appointment reminders, helped to simplify his medications, and set him up with his medications in bubble packs. He was palpably relieved, and this act of patient-centered compassionate, and pragmatic nursing not only helped her patient to understand his health conditions better to stay engaged in care but also ensured safe quality care so that he received the correct medication and made his important specialty follow-ups.
