The population she works with is unique as their discharges are classified as difficult. However, Gilda looks at each patient and their family as unique and special.
Gilda is a Unit Based Care Manager. She has been the driving force for dozens of incredibly complicated and time-consuming discharges. The population she works with is unique as their discharges are classified as difficult. However, Gilda looks at each patient and their family as unique and special.
Gilda sees each patient as an individual. She sees each patient as a person, all with their own likes, dislikes, family support, and idiosyncrasies. She maintains close contact with not only the patients but also with their support system. Gilda is a huge advocate for these patients. She works tirelessly with providers to help optimize orders and plans of care.
Gilda provides open and honest communication with patients and their support. She does not make promises she cannot keep, nor does she avoid having difficult conversations.
Gilda makes connections with each of these patients and routinely checks with them about how they are doing emotionally and mentally, not just focusing on their physical needs.
Gilda truly sees the individual, lives that have changed by disease processes or accidents. She meets patients where they are and allows them grace and dignity while they may be mourning of a life that will never be again.
This is especially poignant with our young trauma patients who are now paralyzed or with our elderly dementia population who will never go home again. Gilda respects each patient and family's individual choices about their own care and escalates needs when identified. She works closely with the unit-based leadership to ensure each patient remains a person and is treated as such.
Gilda actively participates and supports the multi-disciplinary rounds process as the evidence practice shows this helps lower the length of stay, reduce readmissions, and improve team collaboration. Gilda is as real as it gets. She has difficult conversations with families and facilities and makes it her daily mission to ensure safe, appropriate, and timely discharges are occurring. She does this while keeping everyone up to date and personally ensuring each individual patient and their family have the ability to participate in their discharge planning. This is incredibly hard work, and she is exceptional at it.
