Brandi located some formula on the unit and drove over an hour to the patient’s home after work to deliver the formula.
We had a patient discharged who needed enteral feeding. Case management set up a company at discharge to provide formula at home. The patient’s husband called two days after she had been discharged and spoke with Brandi. He told her his wife had not eaten since she left the hospital. The company that was supposed to deliver the formula did not have it in stock and would not be able to have any for another two days. Brandi had taken care of this patient during her admission so she knew the importance of her not only getting her tube feed but that the patient needed water flushes to prevent dehydration. Brandi located some formula on the unit and drove over an hour to the patient’s home after work to deliver the formula. She went inside the home to ensure the patient and family knew how to work the Kangaroo pump. She spoke with the family for a while, hooked the patient up to her ordered regimen, and left. She was back at work the next morning at 0645. Brandi exemplifies being a champion for patient care. She went above and beyond to ensure her patient received the proper care she needed to sustain nutrition and prevented hospital admission. Brandi is a true patient advocate and an asset to not only the team but to the organization.
